Community Health Goals and Strategies

As required by the 2010 Patient Protection and Affordable Care Act, the IRS requires all nonprofit hospitals to complete a Community Health Needs Assessment (CHNA) and adopt an Implementation Strategy (IS) to meet identified needs every three years.  

Beyond conforming with federal requirements, Community Health Needs Assessments help stakeholders understand local trends, demographic shifts, and geographic disparities in a broad range of social, economic, and environmental determinants of health; health-related behaviors; well-being, illness, and injury; and death. They are also a source of planning to implement new programs that address identified needs.
